Comments:
An unannounced monitoring inspection was initiated on December 12, 2024 and completed on December 17, 2024 in response to a self-report received by the licensing office on December 12, 2024 relating to behavioral guidance and forbidden actions. The inspector reviewed one (1) child's record, and 3 staff records on-site. The inspection included document review, interviews, observations and tour of the facility. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to procedures for emergencies and are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-550-O
Description: The center shall notify the parent by the end of the day of any known minor injuries.

Staff #1 observed at approximately 9:45am a bruise on the wrist of Child #1 who was playing in the gym with other children. Staff #3 asked the child what happened, and the child stated a friend did it and then resumed playing. No one saw what happened, and the child did not tell the teachers of any incident involving another person. Staff #3 shared the information with Staff #1 but neither staff notified the parents.

Plan of Correction: We have a training scheduled for 12/19/24. We will retrain all staff on the reporting policy regarding incidents and injuries.

Standard #: 8VAC20-780-550-P
Description: The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.

There was no written documentation of a child's minor injury after a bruise was observed on the child's wrist.

Plan of Correction: We will retrain all staff on 12/19/24 on the process of daily health observations and how to complete the incident report on the day an incident occurs or is noticed.
