An unannounced, on-site monitoring inspection was initiated on 12/16/2024 and completed on 12/19/2024. The on-site inspection began at 10:20 AM and ended at 12:20 PM. The inspector reviewed compliance in the areas listed above. There were 69 children present and 18 staff. The inspector reviewed 6 children?s records, and 5 staff on-site. This inspection included document review, tour of the facility and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Standard #: 22.1-289.035-B-2 Description: Providers must obtain a completed national criminal background check prior to the employee's first day of employment.
Staff #5, who had been employed for one day when the national criminal background check was returned.Plan of Correction: In the future all finger prints will be returned prior to hire.
