During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was initiated and completed on December 12, 2024. The on-site inspection began at 10:20 a.m. and ended at 11:50 a.m. The inspector reviewed compliance in the areas listed above. There were 13 children present and 2 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on December 27, 2024. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-4 Description: REPEAT VIOLATION
Prior to employment, the center is required to request background checks from any state in which the individual has resided in the preceding five years.
The center did not request a criminal history record check from one state for Staff #2, who has been employed for five months. The center did not request a criminal history record check and a search of the sex offender registry from one state for Staff #3, who has been employed for four months.Plan of Correction: We built a checklist with in our Employee Documentation module that includes each requirement with the explanation of the item, the date by which it must be completed, and links to all resources needed.
We also had our to employees send a request for criminal history report to the DC government.
Standard #: 8VAC20-780-280-B Description: Hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children.
A bottle of adhesive remover (warning label states to keep out of reach of children) was in an unlocked cabinet in the Junior Kindergarten classroom.Plan of Correction: We provided each teacher and staff member with additional cupboard keys to ensure secure storage of all hazardous substances.
And we installed clear and visible signage near all storage areas reminding staff to lock cupboards at all times.
Standard #: 8VAC20-780-560-G Description: When food is brought from home, the food container shall be clearly dated and labeled in a way that identifies the owner.
Three out of five lunch boxes observed were not dated and labeled in a way that identifies the owner.Plan of Correction: We prepared pre-printed name labels for each student, including all dates they will be in attendance for the remainder of the school year. and shared these labels with families, requesting their assistance in labeling all lunch boxes and other personal items before sending them to school.
