Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:

Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Comments:

An unannounced, on-site monitoring inspection was initiated and completed on 12/11/2024, as a part of the licensure period, The on-site inspection began at 11:46am and ended at 2:15pm. The inspector reviewed compliance in the areas listed above. There were 16 children present and 4 staff. The inspector reviewed 6 children?s records and 5 staff records on-site. This inspection included documentation, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 12/27/2024.

Description:	An employee may work in the child day center provided that (a) the applicant has received qualifying results on a fingerprint-based background check and (b) the applicant is supervised at all times by a person who received a qualifying result on a background check within the past five years. Staff #2 and #5 were observed working alone in classrooms with children in care without central registry results or documentation that the results were requested.

Description:	Each child shall have a physical examination by or under the direction of a physician:1. Before the child's attendance; or 2. Within 30 days after the first day of attendance. Description:	Children under 10 years of age always shall be within actual sight and sound supervision of staff. At approximately 12:10pm staff #5 left 3 two year old children unsupervised during the designated nap time. One of the two children was awake when the staff left the classroom

Description:	Mats and cots used by children during the designated rest period shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges. There was a child napping in the two-year-old room without a top and bottom cover.

Description:	Staff must obtain current certification in cardiopulmonary resuscitation (CPR) as appropriate to the age of the children in care.
