Comments:
An unannounced monitoring inspection was initiated on 11/12/2024 and completed on 11/18/2024 in response to a self-report received by the licensing office on 11/12/2024 relating to Staff and Supervision, and Special Services. The Department requested that the provider complete an IPOC as result of the last inspection related to Staffing and Supervision, and Special Services. The inspector reviewed 1 childs record and 1staff records on-site on 11/12/2024. The inspection included: document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 01/02/2025 submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-340-A
Description: When staff are supervising children, they shall always ensure their care, protection, and guidance. A child was left on the van sleeping after being picked up at school and returning to center.

1. Child 1(age 9) was left on the van after Staff 1 picked the children up from school and arriving back to the center.
2. Child 1 (age 9) was asleep on the van after pick up. Child 1 (age 9) woke up on the van 45 minutes later and went into the center.
3. Staff 1 was unaware that Child 1 (age 9) was still on the van until Child 1 came into the center.

Standard #: 8VAC20-780-340-F
Description: Children under 10 years of age shall always be within actual sight and sound supervision of staff.

1. Child 1 (age 9) was left asleep on the bus left out of actual sight and sound supervision for 45 minutes by Staff 1.

Standard #: 8VAC20-780-580-A
Description: The center that provides transportation, shall be responsible from the time the child boards the vehicle until returned to the parents or person designated by the parent.

1. Staff 1 left Child 1 (age 9) asleep on the van after picking him/her up from school. The center was unaware that Child 1 was on the van until Child 1 woke up and walked into the center.
