Technical Assistance:
Technical assistance was provided in the following areas;
Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 12/27/24. The on-site inspection began at 11:10am and ended at 1:30pm. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 39 children present and 8 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 1/7/24. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-130-E
Description: The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. The immunization documentation for Child #2, who was under 2 years of age, was dated 13 months ago. Updated immunizations were due six months ago.

Standard #: 8VAC20-780-270-A
Description: The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. On the playground there was a anchor pin on the sand box edging that was lifting out of the edging and protruding approximately 1 1/2 inches. The protruding anchor pin poses a skin snag or clothing entanglement hazard. There was a piece of rotting wood approximately 8-9 inches in length hanging from the roof of the covered area on school age playground.

Standard #: 8VAC20-780-420-E-3
Description: Staff shall request at least annually parent confirmation that the required information in the child's record is up to date. Up-to-date information is required to be requested from the parent for each child's record at least annually. The information in the record of Child #4 had not been updated in over 26 months which is overdue by 14 months.
