An unannounced inspection was initiated on November 22, 2024 and completed on December 31, 2024 in response to a complaint received by the licensing office on November 13, 2024 relating to allegations of forbidden action, behavioral guidance, physical plant and care, protection and guidance. The inspector reviewed one staff record on-site and four children's records electronically on 12/04/2024. This inspection included document review, tour of the facility and interviews.
The preponderance of evidence gathered during the investigation supports the allegation of forbidden action; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and a violation was documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on January 8, 2025. A POC submitted after this date will not appear on the public website.

On October 31, 2024, Staff #1 lifted Infant #1, age 10 months, off the ground by one arm at the bicep and carried the infant by the one arm to a rug a few feet away. Staff #1 also lifted infant #2, age 10 months, off the ground by one arm at the bicep and carried the infant by the one arm across the classroom.
