During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated on December 20, 2024 and completed on December 20, 2024. The on-site inspection began at 1:30 PM and ended at 4:25 PM. The inspector reviewed compliance in the areas listed above. There were 81 children present and 15 staff. The inspector reviewed eight children?s records and seven staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on January 15, 2024. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 22.1-289.035-A
Description: The center is required to have employees complete a background check every five years.

The updated central registry check results in staff #7?s record was obtained three days late.

Standard #: 8VAC20-780-160-A
Description: Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with the children.

The record for staff #1 had documentation of screening completed two days after the date of employment.

Standard #: 8VAC20-780-270-A
Description: The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

1.On the toddler playground, a picnic table had broken plastic seats which poses a risk for pinched skin.
2. On the school age playground, the basketball stand had two uncovered anchor bolts.
3. In pre-k bathroom the toilet seat was cracked and in the room a cover for an outlet was coming away from the wall.
4. In the two?s room, the toilet seat was lose and able to move side to side.
5. In the infant room, the soap dispenser was lose and able to move side to side.

Standard #: 8VAC20-780-500-B
Description: Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

In the two?s bathroom, a foot operated storage system used for disposing diapers was inoperable.
