During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced monitoring inspection was initiated on 12/18/2024 and completed on 01/08/2025 in response to a self-report received by the licensing office on 12/18/2024 relating to staffing and supervision, special services and staff training. The inspector reviewed 1 child record and 1 staff record on-site. This inspection included document review, tour of the center, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 01/15/2025 . A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-340-A Description: When staff are supervising children, they shall always ensure their care, protection, and guidance.
1. Staff #1 confirmed that on 12/17/2024, Child #1 (age 3 years) was served food at lunch time, exposing the child to an allergen.
2. Child #1 was sitting at a table designated for children with food allergies. Staff #1 did not verify the list of allergens that was posted in the classroom before serving food to Child #1.
3. Child #1 suffered an allergic reaction, which included coughing, vomiting and difficulty breathing and was administered emergency medication and was transported to the local emergency hospital.Plan of Correction: 1.Training
Staff (Support Teacher) involved in incident will complete a 1-hour online training, Understanding and Managing Allergies in Child Care, offered through Care.com.

A red cup has been given to the child to use at morning snack and Lunch, when milk is offered {DATE OF CORRECTION: 12/30/2024).
A child-size pitcher, designated with the child's name and for child's use only, is provided to the child to allow him to pour his own milk (DATE OF CORRECTION: 12/18/2024). A specified area of the counter is outlined in red indicating ALLERGY ALERT- NO DAIRY

3.Administrative Corrective Action
LCC's Allergy/Anaphylaxis Policy will be updated to reflect the new online training requirement for all current and new staff hired after January 2025, and environmental corrections reflected in this document. This will be applied to children center-wide who have identified food allergies (DATE OF CORRECTION: 01/24/2025).
Standard #: 8VAC20-780-560-P Description: A child with a diagnosed food allergy shall not be served any food identified in the written care plan.
1. The written care plan for Child #1 (age 3 years) identifies a food allergy.
2. Staff #1 knew that Child #1 was sitting at a lunch table designated for children with food allergies, but was not aware that the child had a separate pitcher to use and
refilled the child's cup using the pitcher used for all children.
