During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced monitoring inspection was initiated and completed on 1/08/2025 in response to a self-report received by the licensing office on 1/03/2025 relating to a serious injury that occurred at the center. This inspection included document review, tour of the facility, interviews and observations.

The information gathered during the inspection determined no violations with applicable standards or law regarding the serious injury. Though not a part of the original self report, violations that were not reported were found related to document posting requirements, building maintenance and physical plant requirements and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 1/15/2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 22.1-289.011-F
Description: The license and any other documents required shall be posted in a conspicuous place on the licensed premises.

The findings from the most recent inspection, dated 11/18/2024, were not posted in a conspicuous place on the premises.

Plan of Correction: The inspection findings from
11/18/2024 will be immediately posted in a conspicuous location on the premises. A routine check will be implemented to ensure ongoing compliance with posting requirements.

Standard #: 8VAC20-780-270-A
Description: Repeat Violation
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

The Toddler classroom had eight metal bolt tops protruding out of the wall and a sharp metal phone/cable outlet without a wall plate.

Plan of Correction: Emergency work orders were submitted on 1/9/2025 to remove the protruding metal bolt tops and the sharp metal phone/cable outlet in the Toddler classroom. A routine inspection will be implemented to ensure that all areas and equipment are maintained in a clean, safe, and operable condition.

Standard #: 8VAC20-780-290-A-3
Description: In areas used by preschool age or younger children, electrical outlets shall have protective covers.

The school age classroom had children three years old to six years old in care. The classroom had six unused electrical outlets that were not protected with outlet covers and were within their reach.

Plan of Correction: Protective outlet covers will be immediately installed on the six unused electrical outlets in the school age classroom. A routine inspection will be conducted to ensure that all electrical outlets in areas used by preschool age or younger children are properly covered.
