During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced monitoring inspection was initiated on 01/09/2025 and completed on 01/09/2025 in partnership with an outside agency in response to a self-report received by the licensing office on 12/20/2024 relating to inadequate supervision. The inspector reviewed one child's record and one staff record on-site. This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 01/22/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-40-E Description: A center is to ensure the activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.
On page five of the Staff Handbook it states "The children will be signed in again by staff upon arrival to M.C.?s Kids in the afternoon. If a child is not received off the van and is scheduled to attend the center, report this to the director/supervisor immediately. Call the parents and the school to verify the child is safe with parents or school." The center failed to immediately report the child not signed in to the director. Child #1 was not checked in on the van attendance log but not checked into the building sign in sheet.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-340-A Description: When staff are supervising children, they shall always ensure their care, protection, and guidance.
On 12/9/2024, Staff #1 left Child #1 (age 6) asleep and unattended in the locked school van upon arrival back to the center. Staff #1 miscounted during headcount and the child was left in the van from approximately 4:10pm-5:07pm. The van was located at the front of the building and the temperature on 12/9/2024 was approximately 55 degrees Fahrenheit.
