During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-790-540-B-3 Description: Centers are required to obtain the name, address, and telephone number for each custodial parent's place of employment. The record for Child #1, who had been attending the center for 5 months, did not contain documentation of the address of each parent's place of employment. The record for Child #3, who had been attending the center for more than one year, did not contain documentation of the address of one parent's place of employment. The record for Child #4, who had been attending the center for more than two years, did not contain documentation of the address of each parent's place of employment. Plan of Correction: Parents were contacted during the inspection. Two parents provided the requested information during the inspection.
Standard #: 8VAC20-790-600-F Description: Staff who work directly with children are required to complete the department's health and safety update course annually. The annual training for Staff #5 is more than one month overdue. Plan of Correction: Staff is in the process of completing the training and will complete the training by 1/15/2025
