The information in the record of child #3 had not been updated in over 14 months, record for child #4 had not been updated for over 15 months, record for child #6 had not been updated in over 4 years.Plan of Correction: Director and staff will ensure that children's records are reviewed annually with parents.

The center's written emergency plan was last reviewed/updated 2022.Plan of Correction: Emergency plan is in the process of being updated. Will be reviewed with staff upon completion on 2/17/25.
Standard #: 8VAC20-780-550-E Description: Shelter in place procedures shall be practiced a minimum of twice per year.
