During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

A monitoring inspection was initiated on December 12,2024, and completed on January 2,2025, as a part of the licensure period. The inspector reviewed compliance in the areas listed above. The inspector reviewed four staff records electronically on December 12, 2024. Additional documentation was provided on January 2, 2025.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on January 23, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-2 Description: Providers must obtain a completed national criminal background check prior to the employee's first day of employment.
The record of caregiver #3, contained a national criminal background check dated one day after the date of employment.Plan of Correction: Per the administrator: Due to an error, this employee received their offsite hire orientation before their prints returned. They were not allowed at the program until fingerprints were returned.
Standard #: 8VAC20-780-160-C Description: A tuberculosis (TB) screening is required every two years from the last screening.
Staff #4 updated screening was due 4 years. There was no documentation of an updated screening.Plan of Correction: Per the administrator: Staff indicated they completed the screening but could not locate paperwork. Weill resend employee for screening.
Standard #: 8VAC20-780-240-A Description: Staff shall complete the VDOE sponsored online orientation course within 90 days of hire.
Staff #5, who is working with children, has not yet completed this course which is overdue by 3 weeks.Plan of Correction: Per the administrator: VDSS preservice training has been completed.
Standard #: 8VAC20-780-245-A Description: Staff shall complete a minimum of 16 hours of training annually, appropriate to the age of children in care.
1. Staff #1 obtained eight of the 16 required hours for the 2023-2024 period.
