During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was initiated and completed on 1/15/25. The on-site inspection began at 10:40am and ended at 12:40pm. The inspector reviewed compliance in the areas listed above. There were 16 children present and 4 staff. The inspector reviewed 3 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 1/24/25. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-80-A Description: The Center is required to keep a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs.
Evidence: During inspection in the Rainbow 1 classroom, there were 12 children present, but the written record reflected there were 16 children present.Plan of Correction: The Center Manager reviewed and
emphasized the importance of maintaining
attendance records with the staff. An
updated daily attendance doucmentation
method has been created to confirm that the
arrival and departure of each child is
carefully monitored throughout the day.
Standard #: 8VAC20-780-280-B Description: The Center is required to keep hazardous substances in a locked place using a safe locking method that prevents access by children.
Evidence: In the bathroom area where the door was unlocked, 3 containers of toothpaste containing labels that stated, ?Keep out of Reach of Children? and the word ?Warning? on the labels were stored on an open shelf.Plan of Correction: The Center Manager reminded all staff
members to keep hazardous substances,
including toothpaste, locked in a cabinet and
out of reach of children.
Standard #: 8VAC20-780-340-F Description: (Repeat Violation) The Center is required to ensure that children under 10 years of age always be within actual sight and sound supervision of staff.
Evidence:
During inspection in the Rainbow 1 classroom, there were 2 teachers located on the far side of the classroom when a child exited the classroom through a gate and entered the hallway, then entered into the bathroom area behind another half wall where they were out of sight and sound supervision.Plan of Correction: Center Manager reminded all staff to position
themselves carefully around the classroom to
ensure that the children are always under
actual sight and sound supervision throughout
the day.
Standard #: 8VAC20-780-530-A Description: The Center is required to have at least one staff member in each classroom or area where children are present that had obtained CPR and first aid training.
Evidence: The file for Staff #1 did not contain documentation for completion of CPR and first aid training.Plan of Correction: The Center Manager will ensure that all
recently hired staff members who work
directly with the children complete an
overview of First Aid and CPR techniques
and become certified.
