Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site renewal inspection was initiated on 01/16/2025 and completed on 01/16/2025. The on-site inspection began at 11:00 a.m. and ended at 1:10 p.m. The inspector reviewed compliance in the areas listed above. There were 53 children present and 9 staff. The inspector reviewed 5 children?s records, 5 staff records, and 2 agent/applicant records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #:	8VAC20-780-160-A
Description:	Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff 3 TB was dated six months prior to date of employment.
Plan of Correction:	Provider will obtain within the required timeline.
Standard #:	8VAC20-780-40-K
Description:	The center shall develop written procedures for prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices and sudden infant death syndrome awareness. The written procedure for prevention of shaken baby syndrome did not include safe sleeping practices.
Plan of Correction:	Provider will update procedure.

Description:	Staff are to complete the VDOE sponsored online orientation course within 90 days of hire. Staff 2 completed the course 24 days late and Staff 4 completed the course over one month late.
Plan of Correction:	Provider will obtain within the required timeline.
Standard #:	8VAC20-780-270-A
Description:	The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. Chipped paint was observed in many areas on the rails of the crib assigned to Child 2.
