During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was conducted on 1/22/2025. The on-site inspection began at 10:34 am and ended at 1:20 pm. The inspector reviewed compliance in the areas listed above. There were 45 children present and 10 staff. The inspector reviewed 5 children?s records and 5 staff records on site. This inspection included document review, tour of the facility, interviews, observations, and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 1/29/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-140-A Description: REPEAT VIOLATION
Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.
There was no physical examination in the record of child #2 and child #4, who had both been enrolled for over 5 months.Plan of Correction: Management has contacted the families of both students and requested current physical examination documentation. Families have a deadline of Friday, January 31, 2025, to comply or services will be suspended until the documentation has been provided.
Due to this violation being a repeat offense, ownership is reassessing the delegation of managerial responsibilities. It has also promoted the current Assistant Director into a Co-Director role with the current Program Director. Ownership and management will collaboratively review current roles and responsibilities and make adjustments to ensure a higher level of accountability, while also tweaking systems to safeguard documentation collection from human error as much as possible. Maintaining current documentation is a critical component of childcare and violations are not taken lightly.
Standard #: 8VAC20-780-520-C Description: If diaper ointment or cream is used, written parent authorization noting any known adverse reactions shall be obtained.
Child #5 did not have written parent authorization for their diaper ointment.Plan of Correction: The child?s family completed a form to provide permission and instructions for using diaper ointment. In investigating the matter, a teacher in the classroom communicated that the family did not receive a form to complete due to losing sight of the necessary documentation while completing several tasks at the moment of drop-off. Management has reiterated the importance of having a signed form provided by the family for each student needing any type of OTC topical ointment including but not limited to diaper cream. Management has ensured that both offices have the document readily available, and the classroom also has blank forms for any families that bring in topical products for use on students.
Standard #: 8VAC20-780-550-P Description: The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include the following:
1. Date and time of injury; 2. Name of injured child; 3. Type and circumstance of the injury;
4. Staff present and treatment; 5. Date and time when parents were notified; 6. Any future action to prevent recurrence of the injury; 7. Staff and parent signatures or two staff signatures; and 8. Documentation on how parent was notified.
An injury report for child #9 was missing one required signature, the date the parent was notified, and how the parent was notified.Plan of Correction: The Regional Director will review the policies involving the completion of Accident and Incident Reports with the managerial team and staff. Management is fully aware of the expectation that every single line must have a response, and if that line is not applicable, then N/A must be inserted accordingly. Management will be responsible for checking every report filed at the end of the day to ensure all aspects of documentation have been completed appropriately. Violations of this company policy will involve repercussions moving forward if necessary.
