During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. As required on the Special Order, the Notice of Intent, Violations Notices attached to the Notice of Intent and the Special Order must remain posted in a prominent place at each public entrance of the center for a period of 12 months from the effective date of the order.
Discussed maintaining documentation of completion of observations by managers at least once per month in which transitions and active supervision will be observed as stated on your Plan of Correction on your previous violation notice. Discussed maintaining documentation that all center staff will participate in monthly training which will be based on information gathered during management observations as noted on your Plan of Correction on your previous violation notice.

An unannounced, on-site focused inspection was initiated on January 28, 2025 and completed on January 28, 2025, as a part of probation. The Department issued a Special Order on January 22, 2025 to place the licensee on probation for a period of three months and require the licensee to contact parents, guardians, or other responsible persons regarding health and safety violations as a results of high-risk violations. The on-site inspection began at 9:36am and ended at 12:32pm. The inspector reviewed compliance in the areas listed above. There were 121 children present and 22 staff.This inspection included document review, tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
