During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site monitoring inspection was initiated on and completed on January 22, 2025. The on-site inspection began at 11:50 and ended at 1pm. The inspector reviewed compliance in the areas listed above. There were 18 children present and 6 staff. The inspector reviewed 6 children?s records and 3 staff records on-site. This inspection included document, review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-140-A Description: Each child shall have a physical examination by a physician before the child's attendance or within 30 days after the first day of attendance.
Child B had no documentation of a physical examination since enrollment last fall.Plan of Correction: I will get the physical from the parent.
