During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 1/16/2025, as a part of licensure period. The on-site inspection began at 9:30am and ended at 1:20pm. The inspector reviewed compliance in the areas listed above. There were 64 children present and 16 staff. The inspector reviewed 5 children?s records and 5 staff records on-site and electronically on 1/21/2025. This inspection included document review, observation, and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 2/6/25. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: For staff hired before July 1, 2024, providers must obtain the results of a central registry search for an employee, within 30 days of service.

Staff #1 was a previous employee that was rehired for almost 2 years. Results of the new central registry search were dated over a year after the rehire date.

Plan of Correction: This was under the old Human Resources, and this has been corrected moving forward.

Standard #: 8VAC20-780-160-C
Description: A tuberculosis (TB) screening is required every two years from the last screening.

Staff #4 updated screening was due 5 months ago. The updated screenings have not yet been completed.

Standard #: 8VAC20-780-280-D
Description: Cleaning and Sanitizing materials shall not be located above food and shall be stored in areas physically separate from food.

Infant 3 classroom had cleaning supplies and food stored on the same shelf, next to each other.

Plan of Correction: The Director and Program Director has spoken with staff about not storing cleaning/sanitizing material where food is kept.

Standard #: 8VAC20-780-530-A
Description: At least one staff in each classroom or area where children are present shall have current certification in both CPR and first aid.

Staff #1 was alone with a group of nine preschool age children and was not currently certified in CPR or first aid. The certification was overdue by 2 weeks.

Plan of Correction: Staff #1 this has been corrected
