An announced, on-site monitoring inspection was initiated on 1/28/2025 and completed on 1/28/2025, as a part of the conditional licensure period. The on-site inspection began at 10:57AM and ended at 1:08PM. The inspector reviewed compliance in the areas listed above. This inspection included document review, tour of the facility and measurements.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-270-A Description: Areas and equipment of the center, inside and outside, shall be maintained in a safe condition. An outlet in the 1-2 years class was not secured flush with the wall at the bottom of the outlet. Plan of Correction: Provider will ensure outlet is mounted flush with the wall.
