63.2 Child Abuse & Neglect
During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced inspection was initiated on January 9, 2025, and completed on January 22, 2025, in response to a complaint received by the licensing office on January 9, 2025, relating to allegations of the temperature of the physical plant. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non- compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the
close of business on February 11, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-270-C Complaint related: Yes Description: In inside areas occupied by children, the temperature shall be maintained no lower than 68?F. On multiple days in early January the temperature was observed ranging between 58 to 66 degrees in rooms while children were in care. Plan of Correction: On February 9, 2025, we noticed that the temperature in two of our classrooms began to drop significantly, while other rooms remained above 80 degrees. Recognizing the urgency of the situation, we promptly contacted Kellam for assistance. They arrived on January 10th and informed us that we needed a heat kit. After approving the job, the issue was initially resolved, and the rooms remained over 70 degrees until January 22nd, when the temperature began to drop again.
We called Kellam once more, and your technicians returned, informing us that the problem had changed but that they would address it. However, after expressing our grievances at the corporate office, we were advised that Kellam was not qualified to service corporate buildings and recommended that we seek a company with more expertise in this area.
The problem was corrected within hours on both dates.
Standard #: 8VAC20-780-340-A Complaint related: Yes Description: When staff are supervising children, they shall always ensure their care, protection, and guidance. On multiple days in early January, children remained in care with temperatures ranging from 58 to 66 degrees in the classroom. Multiple staff members kept children in their coats all day and placed extra blankets on them to keep them warm during naptime. The center?s staff opted not to move the children to other areas of the building that had temperatures of 68 degrees or more. There was no formal notice to the parents of children being cared for in rooms with temperatures lower than 68 degrees. Plan of Correction: The staff were instructed to move our students to the side of the building that was above 70 degrees on January 21st. Children were immediately moved, parents were called, and the center was closed until the unit was repaired correctly.
