During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 1/29/2025, as a part of the licensure period. The on-site inspection began at 2:10 PM and ended at 4:15 PM. The inspector reviewed compliance in the areas listed above. There were 22 children present and 3 staff. The inspector reviewed 3 children?s records, and 3 staff records on-site.
This inspection included: document review, tour of the facility, interviews and observations
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 2/13/25. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-40-E
Description: The operational responsibilities of the licensee shall include ensuring that the center's activities are maintained in compliance with the center's own policies.
The center cannot follow their policy for medication administration when:
1. Child #4's allergy action plan indicates the administration of medication in the event of an allergic reaction. The required medication was not on site.
2. The center did not return or dispose of expired medications. (Child #10, Child #6)

Description: Any child for whom emergency medications (such as albuterol, glucagon, and epinephrine auto injector) have been prescribed shall always be in the care of a staff member who is current in Medication Administration Training (MAT).
No staff present at the center was current in MAT certification, with emergency medications on site.
