An unannounced, on-site monitoring inspection was initiated on 01/30/2025 and completed on 02/04/2025. The on-site inspection began at 9:15 am and ended at 1:00 pm. The inspector reviewed compliance with the areas listed above. There were 23 children present and 5 staff. The inspector reviewed 5 children?s records and 5 staff records on-site.
The inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-130-E Description: The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.
Child 5, who has been attending for more than one year, did not have documentation of obtaining of additional immunizations every six months until the child turned 2 years old.Plan of Correction: Spoke with the parent. Reviewing all children's records to ensure they are all in compliance.
Standard #: 8VAC20-780-245-L Description: There shall always be at least one staff person on duty who has obtained within the last three years instruction in performing the daily health observation of children.
There was no one on duty that has obtained instruction in performing the daily health observation of children within the last three years.Plan of Correction: Most staff have completed the online DHO training.

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
The floors and tables of Classroom 6 were littered with trash and food and there were exposed nails in the wooden molding that ran along the base of the wall. A knob on the closet door, located near the entrance to Classroom 3, was missing and exposed the sharp point of the nail; the nail was at eyelevel to a preschool-age child and was accessible to the children entering/exiting the classroom.Plan of Correction: Everything was corrected.

Hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children.
There was a large bag of Quikrete concrete being used to prop a bathroom open in Classroom 6 that was accessible to children.Plan of Correction: It was removed.

In areas used by children of preschool age or younger, electrical outlets shall have protective covers.
There was an uncovered electrical outlet in the women?s restroom that was used by children of preschool age or younger. The electrical outlet was accessible to the children.Plan of Correction: Will have the outlets replaced with tamper resistant outlets.
Standard #: 8VAC20-780-500-A Description: SYSTEMIC VIOLATION
Children?s hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks, washed with soap and running water after toileting, and staff shall wash their hands with soap and running water before and after a diaper change.
Staff 2 was observed changing a child?s diaper. Staff 2 nor the child washed their hands after the diaper change. Staff 2 stated that the children in Classroom 2 did not wash their hands nor use disposable wipes before eating lunch.Plan of Correction: Talked with staff and will retrain them.
Standard #: 8VAC20-780-500-B Description: SYSTEMIC VIOLATION
The diapering surface shall be cleaned with soap and water and sanitized after each use.
Staff 2 did not clean and sanitize the diapering surface after changing a diaper.Plan of Correction: Staff will be retrained. Making poster to post at the changing station and has rearranged the room to make the supplies more accessible.
Standard #: 8VAC20-780-510-P Description: SYSTEMIC VIOLATION
When authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization.
The authorization for Medication 1 expired one month ago. The medication was still on-site.Plan of Correction: The medication was sent home.
Standard #: 8VAC20-780-560-G Description: When food is brought from home the food container shall be clearly dated and labeled in a way that identifies the owner.
The food from home in Classroom 2 was not dated.Plan of Correction: A new policy was implemented that requires the opening staff member is required to label and date all of the food from home upon each child's arrival.
