During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was initiated and completed on 1/29/2025. The on-site inspection began at 10:15 am and ended at 1:20 pm. The inspector reviewed compliance in the areas listed above. There were five children present and two staff. The inspector reviewed five children?s records and four staff records on-site. This inspection included: document review, tour of the facility, and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 2/12/25. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-130-A Description: The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Child # 4 & 5 (in care more than 30 days) did not have documentation that they have received immunizations prior to attending the center.Plan of Correction: Forms have been given to parents of child 4 & 5 with a deadline of Feb. 28th to return.
Standard #: 8VAC20-780-60-A Description: (REPEAT VIOLATION)
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the emergency contacts information, and first and last dates of attendance.
Child #1, (in care six months), child #3 & 5 (in care over 30 days) did not have addresses for the emergency contacts listed. Child # 2, 4 & 5 did not have the first day of attendance listed. Child #3 ( in care six months) did not address allergies.Plan of Correction: All files have been updated and corrected with appropriate information.
Standard #: 8VAC20-780-70 Description: (REPEAT VIOLATION)
For staff hired after March 1, 1996, documentation that two or more references as to character and reputation as well as competency were checked before employment.
Staff #1 & 2 do not have documentation that the references listed in their files were checked before employment.Plan of Correction: Files were reviewed and references were added to staff # 1&2. References were checked.
Standard #: 8VAC20-780-510-E Description: The center's procedures for administering medication shall include duration of the parent's authorization for medication, provided that it shall expire or be renewed after 10 work days. Long-term prescription drug use and over-the -counter medication may be allowed with written authorization from the child's physician and parent.
Child #1 (10 years old) had a parent's written authorization to administer a long term medication dated six months ago. This authorization was not renewed after 10 work days. The center administered the medication for six months without written authorization from the child's physician.Plan of Correction: We have updated the written permission from the parent and will continue to do so every 10 days as needed.
Standard #: 8VAC20-780-510-G Description: Medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.
The center had a prescription medication in a cabinet that was not labeled with the child's name, dosage amount or times to be given.Plan of Correction: Medication has been labeled appropriately and training has been conducted with staff on rules pertaining to medication.
Standard #: 8VAC20-780-510-L Description: Medication, except for those prescriptions designated otherwise by written physician's orders, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children.
An over-the counter medication and a prescription medication was unlocked in a cabinet, accessible to the children.Plan of Correction: Medications have been locked in a cabinet out access to children. Training with staff has been completed on the rules for medication.
