During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
Effective January 1, 2025, the VDOE will begin determining compliance with ? 22.1-289.057 of the Code of Virginia, which is legislation passed by the General Assembly in 2020. The law requires all licensed child day programs, religious exempt child day centers that serve preschool age children, and certified preschools to test potable drinking water. The law requires that programs submit their plans and test results to the Virginia Department of Health Office of Drinking Water (VDH ODW) and the Superintendent. If the results of the testing indicate elevated lead levels, the program shall remediate, retest, and resubmit results to VDH ODW and the Superintendent. There is an additional alternative bottled water option that comes with additional requirements. Comments:
An unannounced, on-site renewal inspection was initiated on December 12, 2024, and completed on December 12, 2024, as a part of the conditional licensure period. The on-site inspection began at 1:00 pm and ended at 4:00 pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and three staff. The inspector reviewed five children?s records and eight staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-70
Description: Repeat
A complete staff record shall be kept for each staff person.
The records for staff #3 did not contain documentation of two or more references as to character and reputation as well as competency that were checked before employment or volunteering.

Plan of Correction: Per the administrator: I will get the references done.

Standard #: 8VAC20-780-240-A
Description: Staff are to complete the VDOE sponsored online orientation course within 90 days of hire. Staff #6, employed for five months, has not yet completed this course.

Standard #: 8VAC20-780-340-D
Description: Repeat
In each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present. Such a program leader shall supervise no more than two aides.
Records were reviewed for the two staff (staff #3 and staff #4) who were regularly assigned to the toddler-preschool classroom and who were the only staff present in the classroom on 12/12/2024. The two staff records did not contained documentation to demonstrate any of the two staff met program leader qualification. Administration stated she did not have a program leader qualified staff member for the classroom.

Plan of Correction: Per the administrator: I have staff working on training to become lead qualified.
