During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation

Technical Assistance:
Potable water testing plan and results submitted to VDH. CPR and MAT certifications are expiring soon.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 2/6/2025, as a part of the licensure period. The on-site inspection began at 10:20am and ended at 1:20pm. The inspector reviewed compliance in the areas listed above. There were 123 children present and 30 staff. The inspector reviewed 10 children?s records and 8 staff records on-site.

This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (date). A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-70
Description: Staff records shall contain name, address, and telephone number of a person to be notified in an emergency.
Staff #2's record did not contain information for emergency contact.

Plan of Correction: The name, address and telephone number of a person to be notified in the event of an emergency has been added to staff #2?s record.

Standard #: 8VAC20-780-500-B
Description: 1. A nonabsorbent surface for diapering or changing shall be used.
Classroom #1 had 6" tear in diapering pad, rendering it non absorbent.

2. The diapering surface shall be cleaned with soap and at least room temperature water and
sanitized after each use.
Classroom #2 did not have sanitizing solution.
Classrooms #2 an #3 did not have soap and water to clean diapering surface.

Plan of Correction: The pad was replaced with another.
Classroom #2 has been provided sanitizing solution.
Classroom #2 and #3 have been provided soap and water to clean diapering surface.

Standard #: 8VAC20-780-510-L
Description: Medication shall be kept in a locked place using a safe locking method that prevents access by children.

Medication in classroom #1 was in a bag hanging in the classroom and not locked.

Plan of Correction: The medication has placed in a locked medication bag.
