During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site renewal inspection was initiated and completed on 02/04/2025. The on-site inspection began at 10:50 AM and ended at 2:00 PM. The inspector reviewed compliance in the areas listed above. There were 74 children present and 14 staff. The inspector reviewed 8 children?s records and 8 staff records. This inspection included:

? interviews, and
? observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on February 14, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-4 Description: Authorize the child day center to obtain a copy of the results of a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
The facility did not obtain results of a search of the child abuse and neglect registry from a state in which staff 2 resided in the previous five years.Plan of Correction: The facility responded with the following:
Ensure that all paperwork is completed before the staff member is hired, including child abuse and neglect. Ensuring that they are also submitted correctly before the staff member begins.
Standard #: 8VAC20-780-130-A Description: The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
The center did not obtain documentation of immunizations for child 7 who has attended for five months.Plan of Correction: The facility responded with the following:
The parent of the student who was out of compliance was contacted the same day and we received the immunization record with the child's name and doctor's signature.
Standard #: 8VAC20-780-140-A Description: Each child shall have a physical examination by or under the direction of a physician
within 30 days after the child has attended.

Standard #: 8VAC20-780-280-B Description: Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
There were two containers of shaving cream with warning labels kept in an unlocked cabinet.Plan of Correction: The facility responded with the following:
Staff have been instructed that shaving cream is a hazardous substance and have been told to ensure that it is put away in a locked cabinet in the future.
Standard #: 8VAC20-780-500-B Description: A non-absorbent surface for diapering or changing shall be used. For children younger than three years, this surface shall be a changing table or countertop designated for changing.
Children under the age of three years are being changed on the toilet or standing in the bathroom.Plan of Correction: The facility responded with the following:
The classroom that was not in compliance now has two changing pads with waterproof covers.
