During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site renewal inspection was initiated on 2/3/2025 and completed on 2/3/2025. The on-site inspection began at 10:30 am and ended at 1:13 pm. The inspector reviewed compliance in the areas listed above. There were 33 children present and 7 staff. The inspector reviewed five children?s records and five staff records on-site on 2/3/2025.

This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 2/17/2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-270-A
Description: REPEAT VIOLATION
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; protruding nails, bolts or other components that could entangle clothing or snag skin.

Pathway off backyard deck leading to the playground, had cement tiles surrounded by a plastic enclosure that was lifting and no longer in tact, presenting as a trip hazard.

On the outside playground, four scooters and two tricycles, made of metal with red pads, had chipped paint and rust on the handlebars, and two of the tricycles also had broken seats. One horse seesaw was cracked on the front seating and a turquoise foot-car was cracked on the seat.

Plan of Correction: The landing at the bottom of the steps are bricks and the dirt around the bricks washed away and compacted. The area will be covered by a mat until it can be filled in with dirt to remove the tripping hazard.

The mentioned toys were removed from the playground that afternoon until repairs can be made or replaced.
