An unannounced monitoring inspection was initiated on 01/16/2025 and completed on 02/18/2025, in response to a self-report received by the licensing office on 01/16/2025 relating to injury reporting and the supervision of children. The inspector reviewed one child?s records and two staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law related to the self-report. Though not a part of the original self-report, violations that were not reported were found related to daily activities, hand washing, and toileting procedures and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 02/25/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-370-5 Description: Staff shall provide awake infants not playing on the floor or ground a change in play space at least every 30 minutes or more often as determined by the individual infant's needs.

Standard #: 8VAC20-780-500-A Description: (Repeat Violation) Staff shall wash hands with soap and running water before and after a diaper change. Staff #1 did not wash hands before or after changing children's diapers in the infant room.
Staff shall wash their hands with soap and running water before feeding or helping children with feeding. Staff #1 did not wash hands before feeding a bottle to a child.
Children's hands shall be washed with soap and running water after toileting and any contact with blood, feces, or urine.
