Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:

An unannounced, on-site monitoring inspection was initiated and completed on January 29, 2025. The on-site inspection began at 12:30 p.m. and ended at 1:10 p.m. The inspector reviewed compliance in the areas listed above. There were 40 children present and 2 staff. The inspector reviewed 5 children?s records and 1 staff records on-site. This inspection included document review, tour of the facility and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on February 18, 2025. A POC submitted after this date will not appear on the public website.

Violations:

Standard #:	8VAC20-780-140-A
Description:	Each child shall have a physical examination before the child's attendance or within 30 days after the first day of attendance. There was no physical examination on file for Child #4.
Plan of Correction:	The physical examination record for Child #4 has been retrieved from the school central office and placed in the child's records.
Standard #:	8VAC20-780-60-A
Description:	Each center shall maintain and keep at the center a separate record for each child enrolled. At the time of inspection, the center was not able to locate a record for Child #5.
Plan of Correction:	The record for Child #5 has been signed by parents and retrieved and placed in child's file.
Standard #:	8VAC20-780-350-B-5
Description:	The staff-to-children ratios for children ages school age eligible up to 9 years are required whenever children are in care. At the time of inspection, there were 40 children (ages: 5 and up) present with 2 staff, exceeding the 1:18 ratio.
Plan of Correction:	Additional staff will be scheduled, and staff schedules will be reviewed and adjusted to ensure ratio is in compliance with the 1:18 ratio.
