During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on February 10, 2025. The on-site inspection began at 9:30am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above. There were 48 children present and 12 staff. The inspector reviewed 7 children?s records and 5 staff records on-site on February 10, 2025. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.057-A
Description: The program is required to submit to VDH and implement a plan to test potable water for lead. The program did not have evidence of a testing plan submitted to VDH and/or the program did not have evidence of the implementation of the required testing plan.

Plan of Correction: The center responded with the following: The center will get water lead testing completed and submit their testing plan to VDH and DOE. Information will be sent via email to the inspector as well.

Standard #: 8VAC20-780-160-C
Description: At least every two years from the date of the initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening. Staff #1 has an expired tuberculosis test of December 1, 2024.

Plan of Correction: The center responded with the following: Staff #1 will obtain a new tuberculosis test or screening today and the results will be placed in the staff member?s file.

Standard #: 8VAC20-780-290-A-3
Description: Electrical outlets shall have protective covers that are of a size that cannot be swallowed by children. A total of four outlets to include the Pre-K room and the Waddler room had exposed outlets.

Plan of Correction: The center responded with the following: The center placed protective covers to the exposed outlets during the time of the inspection.

Standard #: 8VAC20-780-500-B
Description: Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Both the Waddler room and the 2-year-old room did not have a correct storage system for soiled diapers.

Plan of Correction: The center responded with the following: The center purchased two new garbage cans with foot-operated pedals.
