An unannounced, on-site monitoring inspection was initiated and completed on 2/10/2025. The on-site inspection began at 1:15 p.m. and ended at 2:30 p.m. The inspector reviewed compliance in the areas listed above. There were 37 children present and 7 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, a tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-160-C Description: Staff members shall submit the results of a follow-up tuberculosis (TB) screening at least every two years.
Staff member #3s most recent TB screening is dated 6/21/2022.Plan of Correction: I'll ask [staff #3] to get a new TB screening within the next two weeks.
Standard #: 8VAC20-780-280-B Description: Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
Various cleaners were observed in an unlocked cabinet of the bathroom in the Red classroom.Plan of Correction: I will need to order a new lock.
Standard #: 8VAC20-780-290-A-3 Description: Electrical outlets shall have protective covers.
An outlet next to the sink in the Blue classroom did not have an electrical outlet cover.Plan of Correction: Corrected during the inspection.
Standard #: 8VAC20-780-550-G Description: Documentation shall be maintained of emergency evacuation, shelter-in-place, and lockdown drills.
There was no documentation of an emergency evacuation drill for the month of January.Plan of Correction: Corrected during the inspection.
