Comments:
An unannounced, on-site renewal inspection was initiated on February 10, 2025 and concluded remotely on February 19, 2025. The on-site inspection began at 10:50 a.m. and ended at 11:50 a.m. The inspector reviewed compliance in the areas listed above. There were 32 children present and 5 staff. The inspector reviewed 5 children?s records on-site and 7 staff records and 5 agent records electronically off-site. This inspection included document review, tour of the facility, interviews, and observations.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on February 26, 2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 22.1-289.035-A
Description: The center is required to have employees undergo a background check every five years. The most recent fingerprint check, central registry check and sworn statement for staff #1 and staff #3 was overdue by over three weeks.

Description: Staff shall wash hands with soap and running water before and after a diaper change and helping a child use the toilet. Staff #2 did not wash hands before assisting a child with toileting and changing the child's pull-up.

Description: When food is brought from home the container shall be sealed and clearly dated and labeled in a way that identifies the owner. Six water bottles brought from home did not contain a name or date.
