During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated on completed on 2/10/2025, as a part of probation.The Department issued a Special Order on 1/3/2025 to place the licensee on probation for a period of six months; as a result of high-risk violations. The on-site inspection began at 2:50 pm and ended at 4:50 pm. The inspector reviewed compliance in the areas listed above. There were 115 children present and six staff. The inspector reviewed six children?s records and four staff records on-site and electronically on 2/11,14,17,18/2025. This inspection included: document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 2/25/25. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 22.1-289.011-F
Description: The license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.
The Violation Notice dated 12/9/24 was not posted in a conspicuous place at the center.

Plan of Correction: The Violation Notice will be posted on the board at the front desk.

Standard #: 22.1-289.023-A
Description: The Notice of the Superintendents' intent to take action shall be provided by the Department and a copy of such notice shall be posted in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations.
The Notice of Intent dated 12/5/24 was not posted at the center.

Plan of Correction: The Notice of Intent will be posted on the board at the front desk.

Standard #: 8VAC20-780-350-B-5
Description: REPEAT VIOLATION
The required staff-to-children ratio for school age eligible to 9 years old, with the ratio waiver, is 1 staff to 20 children.
Two staff were observed supervising 56 children form 2:50 pm to 3:00 pm. Three staff were observed supervising 93 children form 3:00 pm to 3:26 pm.

Plan of Correction: Effective February 24, 2025, two staff members from the administrative office will be stationed at the BGCME Abingdon site until we are fully staffed.

Standard #: 8VAC20-780-510-I
Description: Prescription medication shall be given to a child only with written authorization from a parent or guardian.
Child #1did not have written authorization on file for a prescription medication that was at the center during the inspection.
Child #5 did not have a care plan signed for a prescription medication that was administered from July to October, 2024.

Plan of Correction: The authorization for Child #1 will be signed and dated by a parent or guardian. Child #2 no longer attends BGCME Abingdon, and the medication will be discarded.

Standard #: 8VAC20-820-120-E-6
Description: A copy of any special order issued by the department shall be posted in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations
The Special Order dated 1/3/25 was not posted in a prominent place at each public entrance of the licensed day care center.

Plan of Correction: The special order will be posted on the board at the front desk.
