During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Technical Assistance was given and licensing standards were reviewed regarding postings and building maintenance.

An unannounced, on-site monitoring inspection was initiated and completed on February 13, 2025. The on-site inspection began at 1pm and ended at 3:10pm. The inspector reviewed compliance in the areas listed above. There were 13 children present and 3 staff. The inspector reviewed 6 children?s records and 4 staff records on-site on February 13, 2025. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 22.1-289.035-A Description: REPEAT VIOLATION
The center is required to have employees undergo a background check every five years. Staff #1 has an expired Sworn Statement Disclosure and central registry check both overdue by 11 months. The most recent criminal background check for Staff #2 was overdue by three weeks.Plan of Correction: The center responded with the following: Staff #1 and Staff #2 will obtain the required background checks and send the results to DOE as soon as they are received. A copy will be kept in their staff file.
Standard #: 22.1-289.057-A Description: The program is required to submit to VDH and implement a plan to test potable water for lead. The program did not have evidence of a testing plan submitted to VDH and/or the program did not have evidence of the implementation of the required testing plan. Plan of Correction: The center responded with the following: The center will get water lead testing completed and submit their testing plan to VDH and DOE. Information will be sent via email to the inspector as well.
Standard #: 8VAC20-780-160-A Description: Each staff member shall submit documentation of a negative tuberculosis screening. The documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. Staff #3 did not have documentation of a negative tuberculosis screening and has been in contact with children at the center since November 20, 2024. Plan of Correction: The center responded with the following: Staff #3 will obtain her tuberculosis screening/test information and provide it to the center. The documentation will be placed in the staff member?s record.
Standard #: 8VAC20-780-60-A Description: REPEAT VIOLATION
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the work phone number of each parent who has custody. Child #1, Child #3, Child #4, and Child #5 do not have a work phone number listed for their custodial parent.Plan of Correction: The center responded with the following: The center will obtain the custodial parent work phone numbers and place them in the children?s record.
Standard #: 8VAC20-780-70 Description: Staff shall have documentation that two or more references as to character and reputation as well as competency were checked before employment. Staff #3 did not have references prior to employment and has been working with children since November 20, 2024. Plan of Correction: The center responded with the following: The center will obtain two or more references for Staff #3 and place them in her record.
Standard #: 8VAC20-780-270-A Description: REPEAT VIOLATION
Areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition. Three cribs, occupied by children, were observed to have loose fitting crib sheets which pose as a choking hazard to children.Plan of Correction: The center responded with the following: During the time of the inspection, the staff changed the sheets to ?pack-and-play? sheets that were form fitting and taunt. Moving forward, the owner and infant staff will ensure that all crib sheets are fitted correctly.
Standard #: 8VAC20-780-440-G Description: Cribs shall be used for children under 12 months of age and for children over 12 months of age who are not developmentally ready to sleep on a cot or mat. Child #1 (age 14 months) and Child #2 (age 15 months) were both observed to be sleeping in cribs. Plan of Correction: The center responded with the following: The center?s staff will speak to the parents regarding the licensing standard. The child will either transition to a cot or the child?s pediatrician will provide the center with documentation that the child is not developmentally ready to sleep on a cot or mat.
