An unannounced , on-site monitoring inspection was initiated on 02/18/2025 and completed on 02/18/2025. The on-site inspection began at 9:30 am and ended at 12:05 pm. The inspector reviewed compliance in the areas listed above. There were 172 children present and 28 staff. The inspector reviewed 11 children's records and 6 staff records on-site.

Standard #: 22.1-289.035-B-4 Description: The child day center shall obtain a copy of the results of a criminal history record information check from any state in which the individual has resided in the preceding five years.
There is no criminal record check on file for staff 1 who has resided in a state outside of Virginia within the past five years.Plan of Correction: The program director stated that she thought the out-of-state central registry and sex offender check, obtained for staff 1, also included a criminal record check. A criminal record check will be requested for staff 1 today.
Standard #: 8VAC20-780-570-E Description: Prepared infant formula shall be dated.
Six bottles of infant formula were not dated.Plan of Correction: All of these bottles were dated during the inspection. Staff will be remined to check bottles when they arrive to ensure the parent has dated the bottle.
