During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was initiated on 02/18/2025 and completed on 02/24/2025. The on-site inspection began at 10:15 am and ended at 11:21 am. The inspector reviewed compliance in the areas listed above. There were 30 children present and 5 staff. The inspector reviewed 5 children?s records and 6 staff records on-site and electronically on 02/24/25. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 03/03/25. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-2 Description: The center is required to obtain a completed national criminal background check prior to the employee's first day of employment.
The criminal background check for staff #4 was received 9 days after the first day of employment.Plan of Correction: We understand the importance of receiving the background check prior to a new employee's first day of employment and in the future will try our best to comply.
Standard #: 8VAC20-780-160-A Description: Each staff member and individual shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment.
The tuberculosis screening for staff #5 was completed 8 months prior to the date of employment.Plan of Correction: Staff #5 had an actual TB Test completed in February 2024. From now on, we will be sure to have staff complete TB screening within 30 days of the date of employment.
Standard #: 8VAC20-780-40-M Description: The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present.
The center did not have a current written list of all children's allergies, sensitivities, and dietary restrictions in each room or area where children are present. Child #1, enrolled for 18 months has a documented food sensitivity. Child #3, enrolled for 2 weeks has a dietary restriction.Plan of Correction: The written list of children's allergies, sensitivities, and dietary restrictions has been posted in each classroom in a confidential manner.
