Comments:
An unannounced inspection was initiated on November 18, 2024 and completed on March 24, 2025 in response to a complaint received by the licensing office on September 26, 2024 and September 30, 2024 relating to allegation of forbidden actions, ratio, supervision, behavior guidance and nutrition services. The inspector reviewed 12 children?s records and 12 staff records on-site on December 13, 2024 and electronically on February 27th, March 3rd and March 7th. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation supports the allegation of forbidden actions, behavior guidance and supervision therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on (4/1/2025). A POC submitted after this date will not appear on the public website.

Description: When staff are supervising children, they shall always ensure their care, protection, and guidance. On September 19, 2024, staff #3 was unaware that child #2(age 3 years) had exited the center building and was in a parking lot for approximately a minute unsupervised. On July 3, 2024 the aftercare staff were unaware that child #1 (age 3 years) had left the multi-purpose room and went to another area of the center.

Plan of Correction: On September 19, 2024, a parent mistakenly opened the door for child #2. That parent and another immediately realized child #2 was unaccompanied by a parent or guardian. Child #2 was approximately 10 adult steps away from the front door. The parent picked child #2 up and returned them to the door within 42 seconds. Parents were notified of this incident on the same day. On July 3, 2024, Child #1 exited the multipurpose room and was brought back by a parent; parents were notified of this incident at student pickup.
A staff member will always be present in the student dismissal area (addressed September 20, 2024), and all doors will remain closed to prevent students from leaving the supervision of staff (addressed July 4, 2024). Professional development has been ongoing (July 5, 2024 to the present) to further train staff on student supervision and securing exit doors.

Description: During the stated hours of operation, there always shall be on the premises when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member with direct means for communication between the two of them. On July 3, 2024, Staff #2 was working alone at the center with five children between the ages of three and four years old for approximately 30 minutes before another staff member arrived.

Plan of Correction: Staff #2 was not alone. The second staff member arrived at their contracted time, but did not clock in since they are salaried; two staff members were present. All salaried staff have since been reminded to always clock in to ensure a verified record of their presence.

Description: Children under 10 years of age always shall be within actual sight and sound supervision of staff. A child in the four- and five-year-old classroom was observed sitting alone and unsupervised in a hallway working on an activity for approximately five minutes.

Plan of Correction: The teacher stated that they normally position the children at an angle, so that they can maintain visual supervision while the students work independently. The teacher will ensure visibility is maintained and communicate this practice to ensure others are aware that the children are being supervised.

Description: In order to promote the child's physical, intellectual, emotional, and social wellbeing and growth, staff shall interact with the child to provide needed help, comfort and support. Staff #1 confirmed that they spoke to child #1 (age 3 years) in a harsh tone while correcting their behavior.

Plan of Correction: Staff #1, who has a close relationship with Child #1's parents, wanted to emphasize the seriousness of leaving adult supervision and reinforce that safety is our top priority. Child #1's parents expressed their gratitude to Staff #1 for how they communicated this. We subsequently met with Staff #1 and arranged for them to attend professional development focused on conveying the severity of such situations in a manner that supports the child's physical, intellectual, emotional, and social well-being.

Description: The following actions or threats thereof are forbidden: Roughly handling a child. Staff #1 confirmed that they grabbed child #1 (age 3 years) by the collar of their shirt and pulled the child close to their face to correct their behavior.

Plan of Correction: Staff #1, who has a close relationship with Child #1's parents, wanted to emphasize the seriousness of leaving adult supervision and reinforce that safety is our top priority. Child #1's parents expressed their gratitude to Staff #1 for how they communicated this. We subsequently met with Staff #1 and arranged for them to attend professional development focused on conveying the severity of such situations in a manner that supports the child's physical, intellectual, emotional, and social well-being.
