During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Monitor the condition of couches in the School Age classroom. Discussed medication authorization forms and allergy plan.

An unannounced, on-site monitoring inspection was initiated and completed on February 19, 2025, as a part of the licensure period. The on-site inspection began at 11:15 a.m. and ended at 1:15 p.m. The inspector reviewed compliance in the areas listed above. There were 93 children present and 18 staff. The inspector reviewed 9 children?s records and 6 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on February 26, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-4 Description: The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.
Staff #4, employed for more than 4 months, did not have documentation of requesting a criminal history or sex offender check from one state.Plan of Correction: Submitting proper documentation for criminal history and sex offender checks.
Standard #: 8VAC20-780-130-E Description: The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.
The record for Child #7, 16 months old, last date of immunizations was over 7 months old and was overdue by one month. The record for Child #8, 11 months old, last date of immunizations was over 7 months old and was overdue by one month.Plan of Correction: Emailed parents of students indicated about receiving updated forms.
Standard #: 8VAC20-780-40-M Description: Repeat Violation
The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential.
The list in Classroom #3 was posted near the top of the cubbies in plain sight. The list in Classroom #4 and Classroom #9 contained sticky notes on top of the cover page identifying the names and allergies/dietary restrictions in that classroom.Plan of Correction: Retraining staff on the confidentiality of our students when it comes to personal information including allergies.
Standard #: 8VAC20-780-560-G Description: When food is brought from home the food container must be clearly dated and labeled in a way that identifies the owner.
Two lunch containers in Classroom #5 were not labeled or dated.Plan of Correction: Reminding staff and parents of proper food labeling.
Standard #: 8VAC20-780-570-A Description: When a child is placed in an infant seat or high chair, the protective belt shall be fastened securely.
Child #8, 11 months old, and Child #9, 12 months old, were observed in high chairs without the protective belt fastened.Plan of Correction: Retraining staff on infant dining and food safety.
