During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced monitoring inspection was initiated on 2/11/2025 and completed on 2/11/2025, in response to a self-report received by the licensing office on 2/6/2025 relating to behavior guidance. The inspector reviewed 10 children?s records and 6 staff records on-site. This inspection included document review, a tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 2/26/2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-40-E
Description: The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

On February 6, 2025, Child #1 (age 22 months) was seen climbing on top of a classroom table. Staff #1 did not follow the center's policies and procedures regarding behavior guidance by inappropriately redirecting Child #1 (age 22 months) off the table.

The center's behavior guidance plan for parents states: "We will offer alternatives to children engaged in undesirable behavior by offering a different toy, suggesting a new activity, engaging the child in an activity with a teacher another child, or by suggesting independent play. The center's employee handbook states, "unacceptable child guidance is defined as certain discipline methods are simply not acceptable at KCE, including: Spanking, grabbing, pinching, or any physical punishment or restraint."

Plan of Correction: Staff #1 has been retrained on positive child guidance policies. Staff #1 has signed a document "final coaching" form about the incident. Staff #1 has a clear understanding of the policies.

Standard #: 8VAC20-780-400-B
Description: Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior.

On February 6, 2025, Child #1 (age 22 months) was seen climbing on top of a classroom table. Staff #1 did not use appropriate behavior guidance to redirect the child off the table.

Plan of Correction: All staff have been retrained on "respect and protect children" policy. signed and dated.
