During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site focused inspection was initiated on 2/18/2025 and completed on 2/18/2025 as a part of the provisional licensure period. The on-site inspection began at 9:15am and ended at 10:40am. The inspector reviewed compliance in the areas listed above. There were 20 children present and 5 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/2/2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 22.1-289.035-B-4
Description: The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.

The center did not request a search of the child abuse and neglect registry or sex offender registry from one state for Staff #3, who has been employed for over a year and was working with children at the time of the inspection.

Plan of Correction: Staff 3 has completed background check. Sex offender check is completed.

Standard #: 8VAC20-780-60-A
Description: **Repeat Violation**
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The record for child #1, enrolled for over 13 months, was missing the following required components: documentation of confirmation of up-to-date information in the child?s record. The record for child #2, enrolled for a year and 9 months, was missing the following required information: documentation of confirmation of up-to-date information in the child?s record. The record for child #3, enrolled over two years, was missing the following required information: the address of 2 designated persons to contact in an emergency, phone number for 1 parent, work phone number for 1 parent, and documentation of confirmation of up-to-date information in the child?s record. The record for child #4, enrolled for over 5 months, was missing the following required information: place of employment for 1 parent and work phone numbers for both parents. The record for child #5, enrolled for over 5 months, was missing the following required information: place of employment for 1 parent and a work phone number for 1 parent.

Plan of Correction: The records of enrollment was completed for Child 1, Child 2, Child 3, Child 4, and Child 5.

Standard #: 8VAC20-780-340-D
Description: The center is required to ensure at least one staff member who meets the qualifications of a program leader or program director is regularly present in each grouping of children.

Staff #5, who did not meet lead teacher qualifications, was in a classroom with Staff #2 (assistant), who did not meet lead teacher qualifications, supervising infants and toddlers for the duration of the inspection. Staff #1, who did not meet lead teacher qualifications, was alone with a group of preschool children for the duration of the inspection.
