During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. An unannounced, on-site monitoring inspection was initiated on 02/14/2025 and completed on 02/14/2025. The on-site inspection began at 11:14am and ended at 1:40pm. The inspector reviewed compliance in the areas listed above. There were 18 children present and five staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review and tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-140-A Description: Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.
There was no physical examination in the record of child #1, who had been enrolled for 18 months.Plan of Correction: Talked with parent on 2/14/25, asked them to get the physical by March 3/25.
Standard #: 8VAC20-780-550-D Description: The center shall implement a monthly practice evacuation drill.
There was no documentation of monthly evacuation drills for October 2024, November 2024, December 2024.Plan of Correction: Make sure to fill in the drills as I do them.
Standard #: 8VAC20-780-550-F Description: Lockdown procedures shall be practiced at least annually.
