During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site renewal inspection was initiated and completed on 2/19/25, as a part of the licensure period. The on-site inspection began at 11:30 AM and ended at 1:25 PM. The inspector reviewed compliance in the areas listed above. There were 86 children present and 20 staff. The inspector reviewed 7 children?s records and 4 staff records on-site.
This inspection included; documentation review, a tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 2/28/25. A POC submitted after this date will not appear on the public website.

Standard #: 22.1-289.035-B-1 Description: Repeat Violation
The center is required to obtain a completed sworn statement prior to the employee's first day of employment.
The sworn statements for Staff A , who is currently working, was not dated prior to their first day of employment. It was overdue by one day.
The sworn statement for Staff C, who is currently working, was not dated prior to their first day of employment. It was overdue by one day.Plan of Correction: The Center Director has made interview packets to include sworn disclosure statements that will be used to be given to potential employees at the end of the interview and have them complete it before they leave the center. If the employee is not hired, the form will be shredded. All future employees will complete the sworn disclosure at the time of the interview.
Standard #: 22.1-289.035-B-4 Description: Repeat Violation
The center is required to obtain background checks for any residence outside the state of Virginia in the which the individual has resided in the preceding five years.
Staff D, employed for more than 5 years since out of state searches were required, did not have documentation of requesting a criminal history check for a residence outside of Virginia within the past five years.Plan of Correction: The out of state background check was sent out on 1/15/25 when the Director reviewed the file and saw that that background check had not been completed when all the other background checks had been completed. The Center received the out of state background check on [date] and has added it to the employee file.
a) Center Director will create a spreadsheet of background check due dates to ensure compliance of renewal of background checks.
b) Assistant Director and Center Director will add dates into Outlook with reminders 6 months before they are due.
Standard #: 8VAC20-780-40-J Description: Injury prevention procedures shall be updated at least annually based on documentation of injuries and a review of the activities and services.
No annual update to the injury prevention plan was available for review.Plan of Correction: Program received technical assistance from licensor with example of the spreadsheet that could be used to document the review of activities and services.
The Assistant Director will enlist the support of a staff member who will be
responsible for documenting the occurrences and work with Center Director,
Assistant director and lead educators to create a documented plan. RM will review the plan at monthly center visit.
Standard #: 8VAC20-780-370-5 Description: Infants, who cannot turn themselves over and are awake, shall be placed on their stomachs a total of 30 minutes each day to facilitate upper body strength and to address misshapen head concerns.
One of two infants required to have 30 minutes of time spent on their stomach daily, had only 25 of the 30 required minutes of time documented for 2/18/25.Plan of Correction: The teacher informed the Center Director that the child?s name was listed with the space for the last five minutes, and the child had that 5 minutes of time, but because it was not documented it could not be proven.
Spreadsheet will be updated to reflect the amount of time as a column on the
spreadsheet not just the times. Assistant Director, Center Director or lead educators will check the spreadsheets at the end of the week and sign-off that the amount of time was documented correctly.
