Comments:

An unannounced, on-site renewal inspection was initiated and completed on 2/13/25. The on-site inspection began at 10:45am and ended at 1:30pm. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 58 children present and 8 staff. The inspector reviewed 8 children?s records and 9 staff records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/6/25. A POC submitted after this date will not appear on the public website.

Standard #:	22.1-289.011-F
Description:	The license and any other documents required shall be posted in a conspicuous place on the licensed premises. The findings from the most recent inspection, dated 9/25/024, were not posted in a conspicuous place on the premises. The most current license, dated 2/23/2023 was not posted in the preschool building.

Standard #:	22.1-289.035-B-4
Description:	The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. Staff #1, employed for more than 12 months, did not have documentation of requesting a central registry check and a sex offender registry check.
Plan of Correction:	The out of state background checks have been requested.

Description:	Providers must obtain a completed national criminal background check prior to the employee's first day of employment. Staff #9 did not have a completed national criminal background check.

Description:	A tuberculosis (TB) screening is required every two years from the last screening. The updated Tb screening for Staff #4 was due 3 months ago. The updated screening has not yet been completed.

Description:	Repeat Violation Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #1 was missing the following required components: Work phone number for both parents and first date of attendance. The record for Child #2 was missing the address for one emergency contact and first date of attendance. The record for Child #3 was missing the work phone number for both parents and first date of attendance. The records for Child #6, Child #7 and Child #8 were missing the first date of attendance.

Description:	Staff are to complete the VDOE sponsored online orientation course within 90 days of hire. Staff #6, has not yet completed this course which is overdue by 10 months.

Description:	Orientation training shall be appropriate to the age of the children in care and include all of the required facility specific topics. Staff #1, Staff #5 and Staff #6 did not have documentation of the required orientation training in the their records.

Description:	Hazardous substances must be in a locked place that prevents access by children. In the two year old classroom, a can of interior paint labeled keep out of reach of children, was in an unlocked cabinet.
Plan of Correction:	Staff fixed the latch on the cabinet to ensure the paint was not accessible to children during the inspection.
