During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

An unannounced, on-site monitoring inspection was initiated and completed on 2/5/2025. The on-site inspection began at 6:27am and ended at 7:00am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 staff. The inspector reviewed 5 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/3/2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-70 Description: The center is required to maintain certain documentation as part of staff's physical on-site file including, an emergency contact, CPR/First Aid card, Medication Administration Training certificate, Daily Health Observation Training certificate, and Preservice Training documentation.
Staff #1 and Staff #2 were present at the time of inspection and did not have any documentation on site.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-245-J Description: When the center has agreed to administer prescribed medications, there shall always be a staff present on site who has Medication Administration Training (MAT).
Three emergency medications were on site at the time of inspection and neither of the two staff present have obtained MAT certification.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 8VAC20-780-510-P Description: When an authorization for medication expires, the facility shall notify parents that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed.
