During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Comments:
An unannounced, on-site monitoring inspection was initiated on 2/20/2025 and completed on 2/20/2025. The on-site inspection began at 10:24 am and ended at 12:30 pm. The inspector reviewed compliance in the areas listed above. There were 57 children present and 10 staff. The inspector reviewed 5 children?s records and 6 staff records on-site. This inspection included document review, a tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/3/2025. A POC submitted after this date will not appear on the public website.

Violations:
Standard #: 8VAC20-780-160-A
Description: REPEAT VIOLATION

The center is required to obtain documentation of a negative tuberculosis (TB) screening at the time of employment and prior to coming into contact with children.

Staff #2 is currently working and has a TB screening that was dated 46 days after their first day of employment.

Plan of Correction: The reason for this error has been discovered and corrected. An extra check has been added to our process to ensure this does not happen again.

Standard #: 8VAC20-780-245-A
Description: Staff shall complete a minimum of 16 hours of training annually, appropriate to the age of children in care.

Staff #4 and Staff #5 obtained 2 of the 16 required hours in 2024.

Plan of Correction: All staff have been reminded about the 16 hour requirement checks have been put into place to avoid repeat.

Standard #: 8VAC20-780-260-B
Description: The center shall obtain annual approval from the health department for meeting requirements for water supply and sewage disposal system.

The center's environmental sanitation inspection is overdue by 2 months.

Plan of Correction: A CORE inspection has been scheduled. A check has been put into place to avoid a repeat.

Standard #: 8VAC20-780-340-D
Description: Each grouping of children shall have at least one staff regularly present who meets lead teacher qualifications.

Staff #1 and Staff #2, who did not meet lead teacher qualifications, were alone with a group of preschool children at the time of the inspection.

Plan of Correction: Two additional staff members are working on lead training to ensure there are enough lead teachers to cover all rooms

Standard #: 8VAC20-780-530-A
Description: At least one staff in each classroom or area where children are present shall have current certification in both CPR and first aid.

Staff #1 and Staff #2 were with a group of eighteen, three -to five-year-old children and were not certified in CPR or first aid.

Plan of Correction: Two staff members are being sent to a CPR/FA class to obtain certification

Standard #: 8VAC20-780-550-G
Description: Documentation shall be maintained for shelter-in-place drills.

There was no documentation for one of the two shelter-in-place drills conducted in 2024.

Plan of Correction: No one was assigned to note in the book when drills are completed. We have now assigned that task.
