During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Technical Assistance:
The inspector discussed the following with the provider: resilient surfacing, program lead qualifications, carbon monoxide detector, and lead testing.

Comments:
An unannounced, on-site monitoring inspection was initiated and completed on 02/24/2025, as a part of the conditional licensure period. The on-site inspection began at 11:00 a.m. and ended at 1:15 p.m. The inspector reviewed compliance in the areas listed above. There were 6 children present and 3 staff. The inspector reviewed 5 children?s records and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-1
Description: The center is required to obtain a completed sworn statement prior to the employee's first day of employment.

There was no sworn statement for Staff 2, who had been employed for over two months.

Plan of Correction: Provider will obtain.

Standard #: 22.1-289.035-B-3
Description: The center must request a search of the central registry prior to the employee's first day of employment and the provisional hire shall not be alone with children until the results of the central registry search have been obtained as required by the Code of Virginia.

Staff 2, who had been employed for over two months, did not have a central registry search requested. Staff 2 was working alone with children.

Plan of Correction: Provider will obtain.

Standard #: 8VAC20-780-130-A
Description: SYSTEMIC VIOLATION
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Child 1, who had been attending for over one week, did not have documentation of an immunization record on file.

Plan of Correction: Provider will obtain.

Standard #: 8VAC20-780-160-A
Description: SYSTEMIC VIOLATION
Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with the children.

There was no screening for Staff 1, who had been employed for over one month and there was no screening for Staff 3, who had been employed for over two months. The screening for Staff 2 was completed was completed 13 days after hire.

Plan of Correction: Provider will obtain.

Description: SYSTEMIC VIOLATION
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The records for enrolled Child 1, Child 3, and Child 4, were each missing several of the required components.

Description: SYSTEMIC VIOLATION
Staff records shall be kept for each staff person that contain all of the required information.

The records for Staff 1, Staff 2, and Staff 3 were each missing several of the required components.

Standard #: 8VAC20-780-240-B
Description: Staff shall complete orientation training prior to the staff member working alone with children and no later than 7 days of the date of assuming job responsibilities.

There was no documentation of orientation training for Staff 1, employed for over one month, Staff 2, employed for over two months, and Staff 3, employed for over two months.

Plan of Correction: Staff will receive orientation.

Description: There shall always be at least one staff member on duty who has obtained within the last three years instruction in performing the daily heath observation of children.

There was no staff member on duty who has obtained instruction in performing the daily health observation.

Plan of Correction: Staff will receive training.

Standard #: 8VAC20-780-530-A
Description: At least one staff in each classroom or area where children are present shall have current certification in both CPR and first aid.

Staff 1 was alone with a group of 4 children, ages 16 month- 2 years, and was not certified in CPR or first aid

Plan of Correction: Staff will obtain first aid and CPR.

Standard #: 8VAC20-780-530-C
Description: There shall be at least two staff members who have current certification in CPR and current certification in first aid on the premises during the center?s hours of operation, on field trips, and wherever children are in care.

There was only one staff member currently certified in CPR and first aid on premises during the inspection.

Plan of Correction: Staff will obtain first aid and CPR.
