During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
- Technical Assistance:

Technical assistance provided for lead qualification evidence in files and ensuring all components are captured in the center's emergency management plan. Water testing verified and completed.
Monitor ceiling vents in classrooms for frequent maintenance and cleaning as needed.

An unannounced, on-site monitoring inspection was initiated and completed on February 25, 2025, as a part of the licensure period. The on-site inspection began at 10:15 a.m. and ended at 12:45 p.m. The inspector reviewed compliance in the areas listed above. There were 71 children present and 12 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on March 5, 2025. A POC submitted after this date will not appear on the public website.

Standard #: 8VAC20-780-160-A Description: Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.
The TB screening for Staff #1 was dated over 1 year prior to the date of employment and has been employed over 3 months.Plan of Correction: -A new negative TB test result was obtained for staff #1 to meet the compliance.
Standard #: 8VAC20-780-40-M Description: Repeat Violation
The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential.
Six out of seven classrooms did not have an allergy list. One classroom had an allergy list that was not current.Plan of Correction: -an updated allergy list was created and distributed to all the classrooms
Standard #: 8VAC20-780-240-I Description: The center is required to document orientation training for staff members that includes the required components.
The documentation of orientation was not complete for Staff #3 and did not include the name or date of the training.Plan of Correction: New orientation training was provided for staff #3 and a copy is provided.
Standard #: 8VAC20-780-270-A Description: Repeat Violation
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.
-Classroom #3 contained chipping paint on the door to the playground.
-Classroom #4, #5, #6, and #7 contained rust around the bottom of the window next to the back door to the playground.
-Classroom #6 contained cracked and chipping wood along the bottom of the interior classroom door.Plan of Correction: -In classroom #3, the chipping paint on the door is fixed.
-The rust around the windows in Classrooms #4, 5, 6 and 7 are fixed.
-Cracked and chipping wood along the bottom of the door in classroom #6 will be fixed in 3 weeks.
Standard #: 8VAC20-780-280-H Description: Repeat Violation
Harmful agents shall be kept inaccessible to children.
Classroom #4 had shaving cream labeled ?keep out of reach of children? in an unlocked bottom filing cabinet drawer.Plan of Correction: -The shaving cream in classroom #4 was used for an art project, it was placed back in the locked cabinet.
Standard #: 8VAC20-780-440-H Description: When using cribs, the center shall ensure there is no more than one inch between the mattress and the crib.
Child #6, 8 months old, was asleep in a crib with the mattress propped up approximately 3-4 inches.Plan of Correction: The infant room staff were retrained to follow safe sleep practices.
Standard #: 8VAC20-780-450-A Description: The center shall ensure that cribs, cots, mats and beds used by children other than infants during the designated rest period shall have linens consisting of a top cover and a bottom cover. Cribs when being used by infants shall have a bottom cover.
Child #7, 12 months old, was asleep in a crib with an unzipped sleep sack and a blanket.Plan of Correction: -The infant room staff were retrained to follow safe sleep practices.
Standard #: 8VAC20-780-500-B Description: The center shall ensure that a nonabsorbent surface for diapering or changing is used.
The diaper changing pad in Classroom #2 had a rip in it, causing it to not be a nonabsorbent surface.Plan of Correction: -The changing pad in classroom #2 is replaced with a brand new one
Standard #: 8VAC20-780-520-A Description: The center shall ensure that over-the-counter skin products shall not be kept or used beyond the expiration date of the product.
Classroom #2 contained one diaper ointment that expired over 7 months ago.Plan of Correction: -It was determined the expired diaper ointment belonged to a child, who is already disenrolled and was removed promptly.
Standard #: 8VAC20-780-520-D Description: The center shall ensure that insect repellent is inaccessible to children.
Classroom # 2 contained a bottle of insect repellent on top of a low children?s shelf by the back playground door.Plan of Correction: -The insect repellent happened to be empty and was disposed of promptly and staff were reminded of safety protocol around children.
Standard #: 8VAC20-780-550-F Description: Lockdown procedures shall be practiced at least annually.
The center did not have documentation of conducting a lockdown procedure.Plan of Correction: -According to Carousel's policy a lockdown drill was conducted but was not documented.
