During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

An unannounced, on-site monitoring inspection was initiated on 02/07/2025 and completed on 2/13/2025, as a part of the licensure period. The on-site inspection began at 8:55 am and ended at 9:40 am. The inspector reviewed compliance in the areas listed above. There were 16 children present and 4 staff. The inspector reviewed 2 children?s records and 2 staff record electronically on 2/13/2025
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard #: 8VAC20-780-160-A Description: Documentation of a negative tuberculosis ( TB) screening must be submitted at the time of employment and shall of been completed within the last 30 days of the date of employment.
Staff #1 TB was dated 12 months prior to date of employment and has been employed for approximately 1 month.
Staff #2 TB was dated approximately 7 months prior to date of employment and has been employed for 1 week.Plan of Correction: We will get new screen.
Standard #: 8VAC20-780-350-B-1 Description: The staff to child ratio for birth to ages 16 months is 1 staff to 4 children.
At the time of the inspection there were 2 staff to 9 children. The classroom was over ratio from approximately 9:20 am- 3:00 pm.Plan of Correction: We had a staff call in with an emergency last minute. Both administrators were in class to maintain classroom. A child is old enough to transition if needed.
Standard #: 8VAC20-780-440-J Description: There shall be at least twelve inches of space between the sides and ends of occupied cribs except where they touch the wall and thirty inches of space between service sides of occupied cribs and other furniture where that space is the walkway for staff to gain access to any occupied crib.
The eight cribs in the infant space were not positioned with at least twelve inches of space between service sides of cribs to gain access to service sides of cribs.Plan of Correction: We are reconfiguring the space. We just moved when we installed the new sink.
